Chana, our Chief Clinical Officer, received her BSN and RN from Adelphi University in 2012, before graduating summa cum laude with her FNP from Monmouth University. She guides Revive Care’s policies for patient care and oversees all of our clinical programs, ensuring that each patient receives treatment perfectly tailored to their needs.
Chana is always there as a resource for all of our providers, offering medical guidance and insight. Chana loves to spend time with her three children.

Dr. Sofer is a double board-certified physician with advanced training in facial plastic and reconstructive surgery and a recognized leader in chronic wound care. A Los Angeles native, he graduated summa cum laude from UCLA and earned his medical degree at UC Irvine School of Medicine, graduating at the top of his class

Dr. Sofer completed his internship in General Surgery at UCLA Medical Center and pursued specialized training at Columbia University, Cornell University, and Memorial Sloan Kettering. He further refined his expertise through a prestigious AAFPRS fellowship in Facial Plastic & Reconstructive Surgery one of only about 40 such fellowship positions worldwide.

As the founder and head of the chronic wound physician group, Dr. Sofer leverages his extensive experience to lead and shape clinical programs, ensuring the highest standards of care and innovation in wound management. His leadership is dedicated to advancing treatment protocols and improving patient outcomes.
