Dov Levin

Dov Levin, CPA, is the Chief Financial Officer at Revive Care. He graduated from Fairleigh Dickinson University with a Master of Science in Accounting. Dov has work experience in healthcare and real estate auditing, tax advising, and financial reporting, as well as healthcare operations. Dov is an enthusiastic singer, known in the office for his habitual humming.

Krista Miller

Vice President of Operations

Krista has an 18+ year history leading large, high-performing clinical teams with a focus on wound care, patient care, compliance, and education. She has a proven track record in operational management, successfully serving as the Vice President of Operations for the Florida, Georgia, and Arkansas branches of Revive Care. Krista focuses on streamlining processes and enhancing service delivery. She loves spending quality time with her husband of 32 years and their children and grandchildren.

Chana Chapler

Chief Clinical Officer

Chana, our Chief Clinical Officer, received her BSN and RN from Adelphi University in 2012, before graduating summa cum laude with her FNP from Monmouth University. She guides Revive Care’s policies for patient care and oversees all of our clinical programs, ensuring that each patient receives treatment perfectly tailored to their needs.Chana is always there as a resource for all of our providers, offering medical guidance and insight. Chana loves to spend time with her three children.

Stephanie Hodges

Stephanie, Chief of Marketing at Revive Care, earned her Bachelor's degree in Nursing from Indiana University, before receiving her MBA. With her extensive experience in various healthcare roles, alongside her legendary relationship and networking management skills, Stephanie spearheads all of Revive Care’s marketing efforts. She takes great pride in her three young adult children and enjoys attending sporting events- Go Colts!

Tammy Tatum

Director of Clinical Education

Tammy is our Director of Clinical Education. She has 27 years of nursing experience, ranging from hospital and home health to hospice and long-term care. Tammy is an active member of several organizations, including the American Nurses Home Association, Arkansas Nurse Practitioner Association, American Association of Nurse Practitioners, and the NAWC Certification Committee. Tammy ensures that Revive Care stays compliant with all education standards, arranging training summits and helping all providers stay up-to-date with the most current information in wound healing. Before finding her true passion in the world of nursing, Tammy worked at a greeting card factory for eight years.

Stephen Kelly

Medical Director

Revive Care’s Florida Medical Director, is a board-certified Emergency Medicine physician with training at Baystate Medical Center in Springfield, MA. Aside from his 15 years in practice, Stephen also has experience in primary care, urgent care, and telemedicine. His involvement in global health includes the completion of an International Emergency Medicine fellowship, as well as time spent volunteering in Mexico and Haiti. Dr. Kelly resides in rural Connecticut with his wife and two sons. He has studied the jazz saxophone and is fluent in Spanish.
